It's not in any of her appearances of issues 14-19, not even 20 when Johnny mentions her. Not in the recaps, either. Not in the recap to 2 from #3, either. Skimming the letter pages (ick), nothing in there either. Sounds like they made it up or asked Way or something.

I always figured his appearance in Wolverine tied with Wolvie's appearance in GR3 57.

Fairly simple. It's established as happening during Punisher War Journal in the storyline running from 71 to 75. Frank gets some money in it, gets on a plane, heads off, it's footnoted, then when he returns, it is also.

They don't look familiar, and the scene seems to imply they're merely generic mental constructs GR fights while hanging around in Danny's mind. So no flashback either, from what I gather looking at it.
