Daredevil Annual v2 #1

This story takes place over the course of several hours on a night "some time ago." Also, Strange's encounter with the Destines in FF Annual #33 is said to have taken place "recently."
Daredevil (a.k.a. "Radar guy")
Lieutenant Webb (first name is probably "Jack" but it's not stated)
"Voodoo guy" (Haitian street person)
Cuckoo (Jasmine Destine/Kay Cera, a.k.a. "Your Majesty" and also appearing as "Voodoo guy")
Mr. Luca (dies)
3 thugs (Arnie, Mario + 1 other)
Bruno
Plastoid robot (possessed by...something) - not the original robot
Dominic Destine (a.k.a. "the guy in the noisy outfit" - never actually named on-panel)
Doctor Stephen Strange
Vincent Destine (as a spirit...Or maybe a demon posing as Vincent's spirit?)
9 other Clan Destine members in the city - BTS (none named - may include Cuckoo in which case there would be 8 other Destines)
There's also a two-page montage of DD's "memory maze" which shows images of various characters he has fought. None of them actually appear so they are irrelevant.
