The 2nd and 3rd stories of the miss-numbered Daredevil Annual #5 (cover is actually numbered #4) are out of sequence. These are Hell's Kitchen stories before The Kingpin destroys Matt's life there. The 2nd story ends with Daredevil hearing the Fatboys' cry for help leading directly into the 3rd story which clearly references the legal clinic and Matt and Karen's presence there. These events don't follow from the main "Atlantis Attacks" story in the 1st feature.
Matt does have what appears to be an unshaven face in the last panel of the 2nd story similar to his appearance in the first story but this quickly disappears in the 3rd story.
The 4th story of the annual is correctly placed agreeing with the listings for Ben Urich, J. Jonah, and Bushwhacker and I would suggest placing those incorrect entries there:
