Both the Earth-616 Hank McCoy and his Age of Apocalypse counterpart are credited for a flashback in A '99... since this flashback is set post-Onslaught, when Dark Beast was outed as an impersonator, wouldn't just Hank-616 be credited?

Yes. At this point, post-Onslaught, the Dark Beast had been exposed as an impostor. That's the real Beast appearing with Archangel and Iceman in this Annual.

Is that the scene where Black Widow wants to recruit her former Champions teammates (Arch)angel & Iceman and invite Beast back to the Avengers? That's after the end of the Onslaught saga and definitely not the Dark Beast.
