pages 1-2 DD has to decide whether he wants an operation. He doesn't know if his radar will still work.
page 3 "Later" He hasn't "slept for days trying to reach a decision" He has a shuriken in the arm.
pages 4-6 After the operation he walks in town.
pages 7-11 He goes to a gym with a Murdock vs Liazi poster and hears noise from the manager's office, Bullseye is beating Don to get Tyrone's address, Bullseye flees while DD recovers from the fight.
pages 12-16 Bullseye arrives at Don's and Cindy Wilson's adress. DD arrives but gets a shuriken in the arm. He deflects a second one but finds out he sent it to Cindy's head and killed her. He wakes up from his nightmare. His dream was apparently a reenactment of the earlier events.
pages 17-19 He decides against the operation. On TV exotic dancer Cindy Wilson says DD saved her life.

This must occur after Miller's first run (as Bullseye works for the Kingin) and indeed after ASM 252 (Spider-Man black costume) and after Bullseye walks again (DD 200) but ideally before the Beyonder gives his sight to DD (#223) since DD claims no experience of what having his sight back will be like. Within these parameters, the placement of DD is arbitrary. Hulk 300 is said to happen between ASM 253 and 254 in Spidey's chronology, which is when he was wearing the symbiote.
However, The Kingpin is in prison and I don't think that was the case. I don't know of a period when all these elements concur (maybe after Back in Black)

The story is Kingpin telling a tale.
Kingpin says how heroes can't keep secret because they feel the need to confide then he confesses to Mr Turner how he framed Mr Ridge so he would seem to be the killer. DD arrests Ridge but then finds he's innocent. So Murdock defends Ridge and states to the media that heroes make mistakes. Kingpin kills Mr Turner so as to maintain the secret.

Note: This occurs after Kingpin discovers DD's secret identity. Kingpin mentions it so much that it must be recent. No mention other than that the events narratd by the Kingpin occurred over the last six months. Given that there is a six-month interval in DD 227 during which he plans Murdock's ruin and Kingpin also spent six months ruining the life of Mr Ridge, I'd place them both together so that Kingpin appears here before the main story in DD 227 (after the prologue)

Based on the reference to i-pods, Kurt Cobain, Jeff Buckley, this is a contemporary story. So I place it just before he gives up his lawyer activity.

A couple of questions:

What is Spidey doing in the photo in DD:B&W 1? If this is original content, it would count as a flashback -- DD:B&W 1-FB. How long before the main story was the photo taken -- earlier that day?

You note that Daredevil's appearance in DD:B&W 1/2 is only in flashback, so his MCP listing would be "DD:B&W 1/2-FB." What is the nature of the FB and when does it occur relative to the main story -- earlier that day?

It looks like Mr. Turner should be included in the character list for for DD:B&W 1/2.
