In this week's MIGHTY AVENGERS #20, there is a flashback at the beginning of the story that will need to get wedged into A 4. In the flashback, after fishing Cap out of the ocean, Hank Pym examines Cap before he comes to. (In the exam, Cap looks like he's wearing a white shirt or gown, but this must have been placed over his costume and removed.) Here's how the chronology of A4 now goes, with various flashbacks and continuity implants published later.
AVENGERS: EARTH’S MIGHTIEST HEROES #1 (19-23)
AVENGERS #4 (4:1-4:4)
MIGHTY AVENGERS #20 -- FB (1:1)
AVENGERS #4 (4:5)
MIGHTY AVENGERS #20 -- FB (1:2-4)
AVENGERS #4 (4:6-15:4)
SAGA OF THE SUB-MARINER #8 (19:5-19:6)
AVENGERS #4 (15:5-15:6)
SAGA OF THE SUB-MARINER #8 (19:7-20:2)
AVENGERS: EARTH’S MIGHTIEST HEROES #2 (1-15)
AVENGERS #4 (16-23)
Then on MA 20 (22:4), we see a photo of Thor, Iron Man, Cap, Hawkeye, Quicksilver, and the Scarlet Witch. I propose that this photo was taken on the occasion of Reed's and Sue's wedding and the appearance should be placed right after MARVELS 2 (33) in the chronologies of the six heroes.

We're doing photos now? Thought one of the MCP rules was that we didn't count those...

I seem to recall the old Index doing this on at least one occasion. I normally don't try to place photos, but this one seemed pretty easy. We're doing photos now? Thought one of the MCP rules was that we didn't count those...

"Photos are taken on a case by case basis. A generic family photo is unplaceable, and really amounts to nothing more than a prop or a decoration, and can be ignored. On the other hand, a photo which shows a specific event which we haven't previously seen is another matter, and should be listed, and treated the same as a flashback. "
