Daken's codename ("Daken") is first given in W:O #12.
W:O #26 reveals that Daken was a cruel childhood nickname, meaning "mongrel", and that his adoptive parents named him Akihiro.
Therefore his real name should be simply Akihiro, and his codename should be Daken.
However, in Dark Avengers #1, Norman Osborn calls him "Daken Akihiro" as a given name, first and last, in a Western style.
Has this Westernized name been used elsewhere? Is this the name that Daken now goes by? Or was this a one-time blip?

I don't know whether this helps, but in Japanese, Akihiro can only be a first name, not a last (family) name.

Japanese names are traditionally rendered with the family name first and the given name second, which might have been what Osborn was doing when he referred to him as "Daken Akihiro". More likely that it was an error, though.
