The first Crusader story in Black Knight #1 was set around the time Richard's men first arrived in Palestine, so that might suggest they precede the bulk of the Fanfare story.

It's not Earth-616 ... I think some Handbook entries have actually made reference to that universe, though. I'd be interested in learning which issues of what titles have new stories, too. More solid confirmation of this (e.g. not just someone describing them that way) came from Generation X '96 (annual), where Synch copied their powers.
