Doctor Strange and the Sorcerers Supreme 12 (1&2 - 29&30)
Is the correct way to interpret this only pages 1-2 and 29-30 or all pages except 1-2 & 29-30?

The Stylebook will answer many of your questions, but quickly:
When a title and issue number is followed by parentheses, the page (or page range) will be in the parentheses. This also means that the character appears in other pages of the story that are separated by other appearances. The ampersand is used to indicate a double-page spread.
In this particular example, the listing could be translated into English as:
Ancient One appears in Doctor Strange and the Sorcerers Supreme #12 in pages 1&2 (a two-page spread) through pages 29&30 (another two-page spread). Presumably somewhere else in the listings, you should see that Ancient One also appears in issue 12, on some other page (or pages) AFTER page 30.
